Once you have selected a new physician to care for you and your family, you will need to have your medical records transferred. This will give your new physician a more complete medical history. You may want to request an additional copy for yourself to keep with your own records.
To transfer health records from your previous physician(s) to your new physician, you must make a written request. Your new physician may have a form for you to sign, or you can use the simple form we've included on the following page.
